Polytrauma represents an association of multiple injuries capable of inducing a systemic response and resulting in a life-threatening condition. Regardless of the different approaches and definitions, the severity of this association, determined by a specific pathophysiology, requires special therapeutic protocols, adapted to the status of the patient. One of the most challenging situations for the trauma specialists is represented by polytrauma with unstable pelvic fractures the stabilization of these fractures is considered a resuscitative measure, but this must be done while avoiding the “second hit” phenomenon. These can be achieved by adapting the treatment to the patient’s systemic response to aggression, which has to be evaluated by objective means. The authors perform a retrospective study in a Level 1 Trauma Centre in order to analyse the possibility of using biochemical markers to asses the status of the polytrauma patients with unstable pelvic fractures and to establish their treatment.

The functional outcome of surgically treated unstable pelvic ring fractures by open reduction, internal fixation

Abstract Background: Unstable Pelvic fracture, a result of high energy antero-posterior compression injury, has been managed based on internal fixation and open reduction. The mode of fixation in Unstable Pelvic fracture has, however, been a subject of controversy and some authors have proposed a need to address the issue of partial breach of the pelvic ring elements in these injuries. This st...

Pelvic fractures in children and adolescents in polytrauma and high-energy injuries.

BACKGROUND This paper is a retrospective analysis of the treatment of pelvic fractures in children and adolescents who sustained polytrauma and high-energy injuries. Incidence varies from 0.2% to 7.5% of all pediatric injuries. Most often, these are isolated stable fractures that do not require hospitalization. However, a group of unstable fractures requiring surgical intervention remains. MA...

Acute Pelvic Fractures: I. Causation and Classification.

Acute pelvic fractures are potentially lethal, even with modern techniques of poly-trauma care. The appropriate treatment of such fractures is dependent on a thorough understanding of the anatomic features of the pelvic region and the biomechanical basis of the various types of lesions. Although the anterior structures, the symphysis pubis and the pubic rami, contribute approximately 40% to the...

Initial management of pelvic and femoral fractures in the multiply injured patient.

The management of polytrauma patients is clinically challenging and requires a multi-disciplinary team approach. The immediate and definitive operative care of fractures represents the optimal treatment for polytrauma patients with orthopedic injuries. Early orthopedic intervention in long bone fractures and pelvic ring injuries has been shown to decrease pulmonary complications, improve hemody...

Pelvic Fractures in Paediatric Polytrauma Patients: Classification, Concomitant Injuries and Early Mortality

PURPOSE OF THIS STUDY To review the characteristics, concomitant injuries and mortality in children with polytrauma and associated pelvic fractures treated in a Level-I Trauma Centre. MATERIALS AND METHODS Between December 2003 and November 2013, 49 children with an Injury Severity Score (ISS) of 16 or greater and a pelvic fracture met the inclusion criteria and were evaluated. The mortality,...
